New showrunner Allen MacDonald will hit the ground running with a major crisis that requires everyone’s help.
When a boat capsizes, Gaffney’s Emergency Department is overrun with patients, so there won’t be a minute to waste.
This has been done so many times on medical dramas that it’s a tired TV trope, but it’s got a fresh slant this time: several newbies are involved in working through the backlog of patients, and they will want to do things their way.

Sadly, Dr. Marcel is not returning, but we are getting two new doctors, and Chicago Med Season 10 Episode 1 spoiler photos suggest they are both heavily involved in the action during the premiere!
The two newbies are:
- Dr. Caitlin Lenox, played by Sara Ramos
- Dr. John Frost, played by Darren Barnett
Lenox is a new attending who intends to make some serious reforms in the ED, which will undoubtedly cause trouble between her and Archer from her first episode, while Frost is a pediatric intern with a secret.
(Interestingly, both characters are played by actors who have experience on family dramas, as Ramos played Hattie Braverman on Parenthood and Barnett once guest starred on This Is Us as a young Jack Pearson!)
Chicago Med Season 10 Episode 1 spoiler photos depict Frost with a young Black girl, who will likely be the patient he focuses on.
It’s not clear if she’s related to the boat emergency everyone else is dealing with or if there will be a second storyline.
However, it’s likely that he will be called away from his focus on this patient to help out with the emergency since the ED will be overwhelmed with patients.
Meanwhile, Lenox will likely come up with all sorts of ideas for how to make the ED more efficient, which Archer will undoubtedly reject.
He’s used to being in charge, and he won’t want to take orders from someone younger than his son!
Mitch and Hannah Drama Incoming!

Chicago Med Season 9 ended with perpetual villain Pawel accusing Mitch Ripley of beating him up.
That story could lead to a split for the sake of drama for two couples.
Charles was already on the outs with Liliana the second after Pawel lobbied his baseless accusation, and now Hannah won’t be happy with Mitch, either.
Chicago Med Season 10 Episode 1 spoilers say that Hannah’s unhappy with Mitch’s secrecy surrounding this issue.
It isn’t that she necessarily believes Pawel’s accusation, but she doesn’t like being kept in the dark.
Mitch likely thinks his troubled friend Scully beat up Pawel and is trying to protect him, but this approach could cost him Hannah forever.
Meanwhile, the investigation into what really happened will get underway. Hopefully, there will be a head-spinning plot twist that will make this annoying story worth it!
Pawel and his nonsense wore out their welcome a long time ago, so this needs to be the last time he eats up screen time.
Fortunately, Chicago Med Season 10 Episode 1 spoilers say the audience will learn the truth about what happened to Pawel by the end of the premiere, though it’s not clear yet if the characters will learn it too.
Hopefully, they will, so that we can get rid of this storyline and move forward. I’m so irritated with this that I wouldn’t mind if Charles and Liliana broke up permanently, either.
Maggie’s Charge Nurse Skills Will Be Put To The Test
MacDonald has promised that Maggie will take her career in a new direction this season, and that will likely begin with the premiere.

This situation will probably leave her questioning her current role and why she’s putting up with so much BS. She also may discover it’s harder to maintain a relationship with Loren than she thought when he’s a trauma surgeon in training.

Archer And His Son Are Presumably Still On The Outs
Archer’s relationship with Sean, which was always fragile, seemingly broke altogether during the Chicago Med Season 9 finale.
Sean resented that Archer had more compassion for a patient’s relapse than he had ever had for Sean’s struggles with addiction and stormed out.
There are no Chicago Med Season 10 Episode 1 spoilers about Archer’s relationship with Sean, so that story will likely wait until another time.
With Archer having to oversee this huge emergency at Gaffney, there probably isn’t much time for him to deal with Sean, but the rift will surely be on his mind while he is working on patients.
